Dear PayPal,

I have received messages from you via e-mail and postal mail informing
me that the business name on our PayPal account does not match the IRS
information for our tax ID number.

I spoke to a PayPal customer service representative on November 15th.
He told me to send a copy of our 501(c)(3) nonprofit status letter via your
website in order to correct the business name on our PayPal account.

limmediately got a message saying that I also needed to submit a
completed W-9 form. I submitted this on your website on that same date,
November 15th.

lam still receiving messages from you via e-mail and postal mail that a
completed W-9 still needs to be submitted. Either there is a backlog in
your system or the website failed to upload the completed W-9 earlier this
month.

So Iam sending a hard copy of the “Certify Account Owner Form” and
the completed W-9 form with this letter.

Please let me know if you need additional information. If you have
questions, you can reach me at # 434-960-7779 or <michael@vadp.org>.

Sincerely,

Michael E. Stone
Executive Director
